The Chronic Condition Service provides structured, practical support for people living with chronic illness, including complex or fluctuating conditions.

Support is focused on building routines that are realistic, planning around symptoms and flare-ups, and creating a clear plan you can follow.

Who this service is for

This service may be suitable if you live with a chronic condition that affects daily function and you feel stuck, overwhelmed, or unsure how to manage symptoms day to day.

It can also be helpful for anyone who supports someone with a chronic illness — for example a partner, carer, employer, or school/college — who wants practical guidance and clearer next steps.

The 5-session package

A structured package designed to create a clear plan and support you to implement it.

Between-session support

Midweek email check-in (brief support and adjustments between sessions).

  1. Session 1: Initial consultation (45 minutes)
    • Understand your current situation and main difficulties
    • Identify priorities and immediate stabilising steps
    • Agree a practical plan for the next 1–2 weeks
  2. Sessions 2–5: Follow-up sessions
    • Review what's working and what isn't
    • Adjust pacing, routines, and symptom strategies
    • Build a realistic plan you can maintain

No. This is independent, practical support that works alongside your existing healthcare team. It does not replace medical care — it complements it.

Can you help with workplace or school adjustments?
